Be Prepared
Having the right documents ensures a fast, smooth visit for every service type.
A valid, government-issued photo ID is required for all services. Examples: driver's license, state ID, passport.
If your employer or attorney sent you, bring the authorization paperwork or donor pass provided to you.
For phlebotomy lab draws, bring your physician's order or lab requisition form for accurate test panel processing.
